We try to be as accomodating as possible with scheduling.  There are times when an established client can "walk-in" without an appointment.  However, we strongly encourage you to call ahead to be sure that there will be time available to care for your needs.

• Wednesday        9 am - 12:30 pm  /  2 pm - 6 pm
• Thursday            1 pm - 6pm
• Friday                9 am - 12:30 pm  /  2 pm - 4:30 pm

If you are coming in for your first appointment with Dr. Meier, be sure to have your insurance information with you. Please, download the following forms by clicking on the title, fill them out now and bring them with you to your appointment.

All Prospective patients should download and fill out the following 4 documents:

If you were involved in an Automobile accident please also fill out both of the following 2 forms in addition to the above 4 forms. 

If you were injured on the job you only need to fill out the Pain Index Form in addition to the above 4 forms.

If you've been injured in an auto accident or on the job, be sure to bring with you any information pertaining to your claim. Such info would include: 

• Claim number and Billing Info
• Other Doctors reports
• X-ray and/or MRI reports
• Claim manager's contact info
• Attorney contact

Be sure to allow for 10 to 15 minutes before your appointment time for paperwork and any other administrative tasks so that Dr. Meier will have the entire scheduled block of time to spend in direct contact with you.
